Thank you for submitting your application form.   You may now continue to the second step: paying dues.

You have two choices for paying dues: mailing a check or paying online with a credit card.   If you chose to pay by credit card you will be directed to the PayPal service. You do not have to have a PayPal account and you do not have to establish a PayPal account to use this service.

To pay by check:

Make out a check for annual dues of $35 for the calendar year payable to HSA-1 and mail it to the following address. Make sure that your name and the message "HSA-1 Membership Application" are on your check.  Then close this window.   You may want to print this page or record the address before closing the window.
